Thank you for your interest in providing input to University of Rochester Medical Center (URMC) researchers through a Community Engagement Studio. The goal of the Studio is to learn from community experts, in order to help guide research design.

Community Engagement Studios are held virtually or in-person. Zoom or location information is provided in advance of each Studio.  

Each Studio will include up to 15 Community Members and last 1.5 hours. This time includes 10 minutes at the beginning for a presentation from researchers. Most of the time in the Studio is spent in facilitated discussion, which includes time for questions, concerns and feedback from participants. As a community member you will be compensated for your time.

The information requested in this form will be kept secure and will not be shared directly with researchers.

By completing this form I understand that the information I provide about me will be kept confidential. I also agree that I will keep confidential individual comments made during the Community Engagement Studio by Community Members or the presenting researchers.
